Employee Personal Device Form

To be filled out every 6 months to obtain a voucher to put a personal device on the GPSB network.

To obtain access to the network with a personal device, staff will need to obtain permission from their immediate manager as well as sign a release form. The technology staff can then provide a network access key. The network access key will be set to expire every SIX MONTHS and the form has to be resigned and a new access key will be provided.

Guidelines for Use of Personal Devices on GPSB networks:

  • The use of personal devices during the school day is at the discretion of the supervisor or principal.
  • The primary purpose of the use of personal devices at school is educational or instructional in nature. Any personal use of the device, i.e. shopping, etc., should take place after school hours or during staff members’ conference period only.
  • Personal devices must not disrupt class in any way.
  • Access to the GPSB networks is filtered in compliance with the Children’s Internet Protection Act (CIPA).
  • Access from personal devices is limited to internet use only. Individuals will not have access to any documents which reside on the school network or servers from their personal devices.
  • Employees bring their devices to use at GPSB at their own risk. Employees are expected to act responsibly with regards to their own device, keeping it up to date and as secure as possible. It is their duty to be responsible for the upkeep and protection of their devices.

    • GPSB is in no way responsible for:

  • Personal devices that are broken while at school or during school-sponsored activities.
  • Personal devices that are lost or stolen at school or during school-sponsored activities.
  • Maintenance or upkeep of the device (keeping it charged, installing updates or upgrades, troubleshooting/repairing any software or hardware issues).
  • Any issue that occurs to the operating system or data (including data loss) of the personal device.

    • I understand the risk associated with bringing any personal device and connecting to GPSB networks and will not hold GPSB responsible in any way for the loss or damage of my device or any of the contents/data/software on the device. I understand that GPSB technology staff has the ability to monitor the activity of the device while connected to any GPSB network. I also acknowledge that failure to comply with the guidelines for use may result in the removal of internet access of the device.
