Before you begin the application process, gather the required documents

  • 1. Child's Birth Certificate and Social Security Card:

    • The person completing the application must be the parent listed on the birth certificate.
    • If not, court-issued custody papers must be submitted.

  • 2. Proof of Residence (one of the following):

    • Louisiana driver's license
    • State-issued ID card
    • Current utility bill with the parent's name and Grant Parish address
    • Current lease or mortgage statement with a Grant Parish address
    • If living with someone else:
      • That person must provide a verification letter in addition to one of the above items.
      • The letter must state your relationship to them and confirm your residency at their address.

  • 3. Income Documentation (one of the following):

    • Two (2) consecutive check stubs for EACH PARENT or CAREGIVER IN THE HOUSEHOLD for the current year (within 2 months from the application date).
    • An official letter from your employer stating:
      • Where parent/guardian is employed
      • Hourly rate of pay
      • The average number of hours parent/guardian works per week
    • Snap/Food Stamps: Must include the child's name and valid effective dates.
    • Social Security Administration (SSA) verification:
      • A statement from the SSA verifying the child listed on the application is a recipient of SSI benefits.
      • Must be accompanied by two current check stubs.
    • Current foster care placement agreement from DCFS
    • Unemployed parents: Submit a letter of support and income documentation from a support source.
    • Families in temporary living arrangements due to loss of housing or economic hardship (homeless):
      • Contact the Grant Parish Early Childhood Network (318) 627-3274 to learn about the LEA-defined procedures for verifying homeless status.

Once you have gathered the required documents

  • 1. Calculate your gross monthly income (total income before taxes)

    • Use the table provided by the Grant Parish Early Childhood Network to determine your income.
